MRI BRAIN (TUMOR) WITH AND WITHOUT CONTRAST
Referring clinician's provided indication for this examination in Epic: * Brain mass or lesion
TECHNIQUE: Multi-sequence, multi-planar MRI of the brain was performed before and after intravenous contrast.
COMPARISON: MRI BRAIN 2024-Jul-10
FINDINGS:
Brain Parenchyma: Redemonstrated postsurgical changes of right frontal craniotomy for resection of oligodendroglioma. The surgical cavity has been interval decreased in size. There is also interval decreased surrounding T2/FLAIR hyperintensity, especially along the medial aspect of the of the resection bed.

Mild enhancement at the resection bed and curvilinear dural enhancement underlying the craniotomy site, which may represent postsurgical changes. Susceptibility effect at the resection bed and underlying the craniotomy site likely represents chronic blood products/hemosiderin staining.

^^^^Probably good, but maaayybe not? ^^^^^
No evidence of acute infarct. Few foci of T2/FLAIR hyperintensity which are nonspecific, may represent mild chronic microvascular ischemic change.
^^^^ok that is not great, need to understand more like how many and how pervasive this is and were there any in the last scan? ^^^^^^


Ventricular System and Extra-Axial Spaces: Interval decreased tiny subdural collection overlying the right parietal convexity now measuring 1 mm (4:19), previously 3 mm. No evidence of midline shift or hydrocephalus.

Extracranial Structures: Arterial flow voids in the skull base are present. Small mucous retention cyst in the left sphenoid sinus. Scattered mild mucosal thickening of paranasal sinuses. Unremarkable orbits. Clear mastoids.

IMPRESSION:
1. Expected evolution of postsurgical changes from right frontal oligodendroglioma resection with interval decreased size of the resection cavity and decreased surrounding T2 FLAIR hyperintensity.
2. Residual enhancement at the resection bed favored to represent postsurgical changes. No evidence of suspicious nodular enhancement. Continued follow-up with MRI is recommended.
